Summary of Benefits
This summary shows how the cost for covered services is shared between you and the Carpenters Health and Security Plan. Review the Summary Plan Description for complete information, including exclusions and limitations. Covered expenses are subject to annual deductible, coinsurance, out-of-pocket maximums.

Enrollment
To enroll in the plan, you must complete an Enrollment Application and return it to Carpenters Trusts. If your family status changes due to marriage or birth of a child, use the Dependent Enrollment Application to add dependents to the plan.

Eligibility
Eligibility for active participants (non-retired) is achieved by accruing employer contributions in a dollar bank. When necessary, COBRA and Self-Contribution can help you maintain eligibility. The Summary Plan Description has complete information, including eligibility requirements for dependents and retirees.

Providers
The plan allows you to choose your provider, whether it's for medical, dental, prescriptions or vision care. Your out-of-pocket cost will be lower using a network provider.

Participant Advocate
Our business office has a Participant Advocate on staff to help plan members access the benefits they are entitled to. Need help finding a provider? Is there a surgery to preauthorize? What about medical equipment needs following release from the hospital? Those are just a few reasons to call.

Programs
Were you recently diagnosed with diabetes, heart disease, heart failure, asthma or COPD? Why not enroll in a free Condition Management program for telephone coaching and support? There is also a program for chronic back pain.
Quit For Life is a tobacco cessation program available at no cost to plan members.

Claims
Medical claims are processed by Carpenters Trusts.
- Contact Customer Service during business hours if you have a question or issue regarding a medical claim.
- Log on to our secure website for medical claims, out-of-pocket expenses and other personal information about your benefits.
With a non-network providers, it may be necessary to file your own medical claim. Claims for prescriptions, dental, vision, time loss, life insurance, and accidental death and dismemberment are processed differently and should NOT be submitted to Aetna.
