Carpenters Rewards

Carpenters Rewards is an incentive program designed to encourage participation in activities that help you improve and maintain your health. Points are awarded for each activity. If you achieve 200 points by December 31, 2012 you qualify for a $200 gift card. Your gift card can be used at 350 stores, restaurants, online merchants, travel destinations, entertainment venues and more.

Health Exam and Biometric Screening (75 points): To qualify for points, participants must submit proof of a health exam and biometric screening by Sept. 30, 2012. Use the Biometric Screening Form and follow instructions.

Mayo Clinic Health Management Programs (25 points): To qualify for points, participants must complete one of the following programs between June 1, 2012 and Dec. 31, 2012:

  • My Fitness Solution

  • My Weight Solution

  • My Stress Solution

My Fitness Solution and My Weight Solution require the participant to track activity once per week for at least six weeks. My Stress Solution requires the participant to track activity once per week for at least four weeks. All programs require completion of a participant survey. Participants may complete more than one Mayo Clinic Online Program as a substitute for Mayo Clinic Health Coaching. Look for these self-guided programs under the My Health tab on the EmbodyHealth website.

Mayo Clinic Health Assessment (75 points): Participants must complete the Mayo Clinic Health Assessment between Sept. 1, 2012 and Oct. 31, 2012 to receive points. The Health Assessment is a confidential online health survey with questions covering 11 key medical and lifestyle risk factors, screening information, medical history, and immunizations. The Health Assessment can only be completed on the Mayo Clinic EmbodyHealth website. Once you are logged on to EmbodyHealth, click the Home page or the Rewards page to find a link to the Health Assessment.

Mayo Clinic Health Coaching (25 points) is a phone-based counseling program that teams you up with a behavior change specialist to help you reach your goals related to fitness, stress, nutrition, or weight management. The Mayo Clinic Health Assessment determines eligibility for Health Coaching. Only participants indicating moderate to high levels of health risk qualify for Mayo Clinic Health Coaching. To enroll, the participant must accept the offer that follows completion of the Mayo Clinic Health Assessment. To qualify for Carpenters Rewards points, participants need to complete 4 calls (moderate risk) or 6 calls (high risk) with a Health Coach and complete a final participant survey between Sept. 1, 2012 and Dec. 31, 2012.

My Coach (25 points): Participants who don't qualify for Health Coaching may participate in an additional online program or use My Coach, a self-guided program for setting healthy goals and tracking progress. My Coach is available on the EmbodyHealth website and can be used for fitness, stress, nutrition, or weight management. To qualify for points using My Coach, just record progress 6 times (once per week) and complete a participant survey.

Participation: Participation is voluntary and open to active and retired participants, spouses, domestic partners, and dependent children (ages 19-25) who are not eligible for Medicare.

Qualification Period: Carpenters Rewards begins in June 2012 and ends Dec. 31, 2012.

Registration: Participants must be registered to use the Mayo Clinic EmbodyHealth website to complete activities and redeem points for the gift card

Redemption: Carpenters Rewards points are awarded and redeemed on the EmbodyHealth website. You do not have to wait until the end of the year to receive your gift card. You will receive your award offer by mail upon achieving 200 points. There is no payout for achieving fewer than 200 points. Gift cards are offered through Hallmark Business Connections. Hallmark will make it easy to redeem your card. With 350 stores, restaurants, travel and entertainment providers to choose from, the hardest part will be deciding where to spend it. For fastest delivery of your card, complete the offer form online (directions provided). Otherwise, return your completed offer form by mail. For added assurance, you'll be able to check your award status online or call a toll-free number.
