Mayo Clinic Online Health Assessment Begins Today With $75 Gift Incentive
If you're ready to take that next step to improve your health, the Mayo Clinic Health Assessment is a great place to start. This interactive, internet questionnaire helps you assess your health strengths and your health risks, as well as what you can do to improve them. This valuable tool is available on our plan's custom website -
www.CarpentersHealth.org - to all non-Medicare participants and dependents age 18 and older who have current eligibility under this plan. Letters were recently mailed to all carpenters and dependents who qualified. Because taking the Mayo Clinic Health Assessment is an important way to help you improve your health, the plan is providing a $75.00 gift incentive when you complete your health assessment. The Mayo Clinic Health Assessment is available beginning April 1, 2010. The last day to take it is May 31, 2010. Please contact Participant Services at the Trust Office if you have questions.
