Introduction

Coverage for you and/or your dependents terminates on the earliest of the dates described in "Termination of Coverage" on page 15. However, coverage can be continued beyond the applicable termination date if you and/ or your dependents qualify for, and properly apply for, one of the continuation coverage options. Two of the continuation coverage options are available on a month-to-month basis at your expense. A third option, Extension of Benefits, is available at no cost to the affected individual. When eligibility in the continuation coverage option terminates, you can regain eligibility only by returning to work for a contributing employer and reestablishing eligibility in the Employee Health Plan.
