Extension of Life Insurance Benefits For Total and Permanent Disability
This Benefit Is For Participants Only
If, while life insurance coverage is in force, you become totally and permanently disabled from an illness or injury before age 70, thereby being prevented from performing any work or engaging in any occupation for wage or profit (and you are not paying for coverage under Self-Contribution Coverage), your life insurance may remain in force as long as you remain totally disabled. Written proof of such total disability must be submitted to the Trust Office within 12 months of loss of eligibility due to such disability. Proof must be satisfactory to the life insurance carrier. In no event will the amount of life insurance exceed the amount in force on the date you lose eligibility except for the participant who is covered under the Carpenters Health and Security Plan—For Retired Carpenters.
