Summary
Vision benefits are based on the maximum dollar amount listed in the Schedule of Vision Care. Routine vision services and supplies are not subject to the $200 annual deductible or the $2,300 annual coinsurance maximum.
Routine vision benefits (exams and vision hardware) are not available to participants and dependents with the Eastern Washington, Idaho, Montana, and Wyoming benefit package.
