Summary Plan Description
Introduction
The Carpenters Health and Security Plan of Western Washington—For Employed Carpenters (also referred to as the "Employee Health Plan" or "plan") is funded by hourly contributions called for under collective bargaining. An employer signatory to a collective bargaining agreement recognized by the Board of Trustees is required to pay an hourly contribution to this plan for each compensable hour. It is, however, your responsibility to make certain the appropriate contributions were made to this plan on your behalf. We encourage you to compare the hours you worked (as noted on your personal work records such as your paycheck stubs) with the hours reported to the Trust Office as indicated on your Benefit Statement. If you find a discrepancy, please contact Participant Services at the Trust Office as soon as possible.
Key Point
Before any claims can be processed, an up-to-date Enrollment Form must be on file at the Trust Office.
