Lump Sum Death Benefit

If you are vested in this plan and your spouse does not qualify for the primary spouse’s benefit or the qualified spouse’s benefit, he or she may receive a lump sum payment of $5,000. If you are not married or do not have minor dependent children, you may designate another person to receive this benefit.
