Tobacco Cessation Program—100%
Benefits are provided for services and supplies to assist with tobacco cessation when you, your spouse, your domestic partner, or your dependent child age 18 and older participates in the plan-sponsored tobacco cessation Quit For Life Program.
QuitForLife is a telephone-based treatment program that combines professional counseling with quitting aids such as nicotine gum, nicotine patches, Zyban, and Chantix. This program is available at no cost to the patient as long as the patient is actively participating in the Quit For Life Program. Please contact the Trust Office for additional information.
Benefits are not provided for services and supplies related to tobacco cessation, except in conjunction with the plan-sponsored tobacco cessation program.
