Summary Plan Description
Termination of Coverage
Your life insurance under this plan continues until the earliest of:
- Termination of eligibility including your death.
- Termination of this life insurance policy.
- Termination of the Employee Health Plan or the Retiree Health Plan.
Your dependents' coverage ends when your eligibility ends or when they no longer meet the eligibility requirements of this plan. The Board of Trustees may, from time to time, elect to change life insurance companies or to self insure certain life insurance benefits.
