Introduction
If the
retiree or spouse is enrolled in the Prescription Drug Supplement, the benefits
described in “Prescription Drug Benefits” and “Life Insurance
Benefits” are the only benefits available under this plan. If the retiree
or spouse is enrolled in the SecureHorizons Medicare Managed Care Plan for group
retirees, the benefits described in “Life Insurance Benefits” are
the only benefits available under this plan.
This plan
provides life insurance protection for the retiree, spouse and dependent
children. This protection is funded, in part, by monthly contributions to this
plan and is administered and paid under a separate insurance contract from an
insurance company authorized to do business in the State of
Washington. Life
insurance benefits are
not
available to retirees, spouses or dependent children participating in COBRA
Continuation Coverage.
