ID Card

The Trust Office provides the retiree with a Carpenters Trusts ID Card that lists the medical review agency’s toll-free telephone numbers. With these numbers, an individual can call the medical review agency from anywhere in the country for assistance with a preadmission certification or a second surgical opinion requirement. This identification card is not a guarantee of eligibility for benefits.
