Termination of Coverage

Your and/or your dependents’ coverage terminates on the earliest of the following dates unless you or your dependents qualify for, and properly apply for, one of the continuation coverage options described under “Continuation Coverage Options”:

  1. At the end of the month in which you or your dependents no longer satisfy the eligibility requirements of the plan. Eligibility for your spouse and stepchildren continues until the end of the month of the date the legal separation or dissolution of marriage became final.
  2. On the first of the month following the last month the appropriate contribution was made for continuation coverage.
  3. On the first of the month your eligibility is suspended.
  4. In the event of your death, eligibility for dependents continues based on your eligibility at the time of death.
  5. The date the Employed Plan is terminated by the Board of Trustees.
