Penalty For Not Using the Second Surgical Opinion Program

If an individual does not satisfy the second surgical opinion requirements for one of the listed procedures, the plan pays the surgeon’s covered charges at 50 percent. The surgical procedure must be medically necessary and covered by this plan.
