Introduction
This plan provides life
insurance protection for you (the participant) and your family. It also provides
you with accidental death and dismemberment protection. This protection is
funded by employer contributions but is administered and paid under a separate
insurance contract from an insurance company authorized to do business in the
State of Washington.
