Please present your Carpenters Trusts ID Card to the provider’s receptionist each time you visit a preferred provider. The ID card verifies your participation in the network, ensures direct billing, guarantees the preestablished discounted rates, and lists the telephone numbers required for inpatient preadmission certification and second surgical opinions. This card is not a guarantee of eligibility for benefits.
