Benefits are provided for the
treatment of organic erectile dysfunction when the individual has a history of
one or more of the following.
Treatment
must be preauthorized with the Trust
Office:
- Prostate
cancer.
- Spinal
cord disease or
injury.
- Insulin-dependent
diabetes.
- Severe
Peyronie’s disease.
Covered
treatment includes vacuum erection devices, injection therapy, penile
prostheses, urethral pellets, and prescription medications. The plan does
not
cover treatment for non-organic impotence
such as psychosexual
dysfunction.
