Eligibility and Enrollment

Summary

You must satisfy certain eligibility requirements to be eligible under this plan. Certain family members are also covered if you meet the eligibility requirements. An Enrollment Card and the required documents must be on file at the Trust Office before a claim can be processed. Eligibility can be suspended or terminated in certain specific circumstances described in this section.
