Health & Security Plan SPD for Active Participants

Benefits are provided for an approved diabetic training program prescribed by a physician, including education, training, meal planning (although not the cost of meals), and treatment and prevention of hyperglycemia and/or hypoglycemia. Coverage is also provided for diabetic supplies including insulin, syringes, injection aids, blood glucose monitors, test strips for blood glucose monitors, visual reading and urine test strips, insulin pumps and accessories to the pumps, insulin infusion devices, prescriptive oral agents for controlling blood sugar levels, and glucagon emergency kits.

If you are diabetic, the following guidelines may be helpful to you when purchasing diabetic supplies:

We recommend that you purchase insulin, syringes, blood glucose testing strips, and lancets through either of the two prescription drug programs – the Medco Health Retail Pharmacy Program or Medco By Mail. If you purchase these items from a nonparticipating pharmacy, you must pay full retail price and submit a Prescription Drug Reimbursement Form to Medco Health. Medco Health will reimburse you at 100 percent of the “average wholesale price” – the program’s discounted price – less the appropriate copayment.
If your physician prescribes a home blood glucose monitor, you may purchase the monitor from the pharmacist and submit the pharmacy receipt with the completed claim form to the Trust Office. Home blood glucose monitors cannot be processed through either of the two prescription drug programs.
