Coverage Following Termination of Eligibility

When lenses and frames are prescribed and ordered before termination of eligibility, benefits are available if the prescribed lenses and frames, or contacts are purchased within 60 days of the date prescribed. Benefits are based on the benefits in effect on the date coverage terminated.
