The Trust Office provides you with a Carpenters Trusts ID Card that lists the medical review agency’s toll-free telephone numbers. You should carry this card with you at all times. With these numbers, you can call the medical review agency from anywhere in the country for assistance with a preadmission certification or a second surgical opinion requirement. This identification card is not a guarantee of eligibility for benefits.
